You will join a team that is passionate about providing meaningful care to at-risk rangatahi. The Rūnanga has a number of homes for rangatahi, and its important that our operations remain safe, innovative and progressive for the best care investment in the young people of the rohe. From front end service delivery to back end compliance, it takes a team to serve the community.

 

Ko te tūranga mahi | About the role

The Care Services Coordinator is responsible for:

·        Care planning & coordination - Facilitate comprehensive assessments with rangatahi and whānau to identify strengths, needs, and aspirations.

·        Monitoring & compliance - identify service gaps and advocate for necessary resources, process improvements and support.

·        Crises support - support immediate response to incidents, injuries, or emergencies.

Well, after hearing about us and about the role, you’ll know whether you’re the right person, and whether now is the right time for you. Your high-level experience and professional expertise in care and emergency services is a given, and you will be excited to engage and learn alongside Ngāti Toa.  Of course, you’ll be well practiced with

·        Maintaining accurate, timely records in accordance with organisational policies and privacy standards.

·        Role modelling kaupapa Māori principles in everyday practice.

·        De-escalation and incident support
